About

This is the application and document section for the Caldwell City Clerk's office. Here you will find applications for various licenses and permits administered by the Clerk's office. Not all formats are currently available for the forms listed below; we will add new formats as they become available.


Form Formats

Fillable and Digital forms must be downloaded to your computer or mobile device before filling them out. Otherwise we will get a blank form.

Digital Submission

Download to computer, fill out, and click 'submit'. You can also add any required attachments to the submission email.

Fillable

  1. Download form to computer/mobile device, and fill out.
  2. Print the form, sign (in front of a notary) the section requiring a notarized signature
  3. Deliver the completed form to City Hall (205 S 6th Ave in Caldwell - blue drop box, if our doors are not open). Or you may mail it to: City Clerk, PO Box 1179, Caldwell, ID 83606.

Print Form

Print form, fill it out by hand, and scan/email to cityclerk@cityofcaldwell.org, or you may deliver the completed application to City Hall (205 S 6th Ave) in Caldwell.

Paper Form

Available at the City Clerk's office (205 S 6th Ave), if the applicant would prefer to pick up a paper form to fill out by hand and submit to City Hall.

Insurance Requirements

Several of the license and permit applications below require submission of liability insurance as part of the application process. Please use the Certificate of Liability Insurance Requirements page when requesting liability insurance from your provider.

Alcohol Catering Permit

(Updated 3.18.2024) Note: If alcoholic beverages are to be served outdoors, a site map must be provided with the application, showing the dispensing area and size of the "beer garden".

  • Cost of the permit is $20/day (max of 2 days for a party & 5 days for a festival event)
  • Applications must be submitted at least 5 business days before the event

Online Payment Process

  1. Go to Payment Portal
  2. Scroll down to the "City Hall" category and select the name of the permit/license. If it is not listed, click on "City Clerk Miscellaneous".
  3. For 'service description', enter a description for the fee you are paying for (example: FBI Background Check). Do the same for "invoice number or description".
  4. Fill out your payment information
  5. For 'amount to pay' enter in the amount from the 'City Clerk Fee Schedule' above
  6. Print a copy of your receipt.
