Step 1.Access the Online Form

1. Go to our online form.
2. Select the appropriate option: New License, Renew License, License Transfer, or Replacement Tag.

Step 2.Fill Out the Form

1. Enter all the required information corresponding to your selection.
2. Ensure each dog has a separate form submission.

Step 3.Submit the Form

1. Review your information.
2. Click Submit.

Step 4.Make a Payment

Electronically

1. Access our secure payment portal.
2. Select the payment option that matches your submission, such as New Dog License or Renew Dog License.
3. Complete the payment process.

By Mail

1. Complete printable form.
2. Prepare Check or Money Order
